By checking the relevant box during event registration, you acknowledge having read, understood and agreed to the below consent, release, waiver and indemnity for the registered individuals.

General

As the parent /guardian of a child under the age of 19, I agree to allow my child/children to participate in Yolo Baseball games, practices and activities. I understand that sports, fitness and recreational activities may involve physical risk including serious injury, property injury, or property loss.  I hereby agree freely and expressly to assume and accept any and all injuries and losses my child/children may incur while participating.

In consideration of Yolo Baseball allowing my child/children to participate, I, for my child/children, myself, my heirs and assignees, hereby release, waive and forever discharge Yolo Baseball, its staff, coaches, contractors, volunteers and affiliates of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect to death, injury, loss or damage to person or property howsoever caused, arising to or by reason of participation in the aforementioned programs, whether as a spectator, participant, competitor or otherwise.

Consent to Treatment

I hereby give my consent to have the registered participants treated by emergency medical personnel, a physician, or surgeon, in case of sudden illness or injury while participating in the above activity.

Photo Release & Waiver

I authorize Yolo Baseball to take and publish photos/videos of my children on Yolo’s social media accounts and website, for use in promotion of Yolo programs.  I release Yolo Baseball from any expectation of confidentiality for the registered participants. I acknowledge that this authorization is voluntary and neither I nor my children will receive financial compensation. I further agree that use of my children’s images does not confer any rights of ownership. I release Yolo Baseball from any liability or claims by me in connection with such photo or video use.
